PLEASE CALL BY THE SUNDAY PRIOR TO THE TUESDAY EXAM DATE.

The test fee, effective Jan. 1, 2009,  is $15.00.

Two (2) pieces of ID, including a photo ID, are required. Please bring an ORIGINAL and PHOTOCOPY of your license. If you are using a CERTIFICATE OF SUCCESSFUL COMPLETION OF EXAMINATION (CSCE), bring the original and a photocopy. 

We are unable to make photocopies at the test site.
